Application Forms
An application form will allow easy access to all necessary information.
The form should include a position title, personal contact details, proof of identity, references, brief summary of work history & experience, languages spoken, medical health limitations, next of kin details, availability, etc. (See Appendix for an example)
The form can be mailed or emailed out in response to any enquiries received along with a CSO information kit, if appropriate (see below).
Arrangements will need to be made for secure storage of all documents containing personal information. A lockable cabinet is sufficient.
Sample Forms
INTRO FORM: Ideal for someone just registering interest in a positon. ie Prior to interview. Limited personal details are collected at this point.
DETAILED APPLICATION FORM: Ideal for completion at interview time when you are likely to take candidate on as a volunteer. This collects more details like next-of-kin, emergency contacts etc..
